Impact WLE Agreement Form 2015-2016. This is the official agreement between you (the mentor), the student, the student’s legal guardian, and Impact academy. All parties must sign before the internship begins.
WLE Timesheet 2016-17. Your intern should ask you to sign this sheet every day to record their hours. Only download this sheet if the original is lost.
Job Shadow Day Permission Slip 2016-17. Students must turn this into their advisor for Job Shadow Day.

Answer all questions in the form one long paragraph (7-10 sentences)
Use full sentences, proper grammar, and spell check.
You will get full credit for giving SPECIFIC details and examples from your WLE day. (Pictures are appreciated too!)
Blogs are due by MIDNIGHT on FRIDAY (Blogs are time stamped, so we know if it’s on time)
Stay on track! If you stay up to date, you will have much less work to do in January.
